We put in the extra work so you're free to grow your practice intentionally
FEWER CALLS TO YOU
We are your 24/7 virtual and telephonic nurse support between office appointments.
LESS SICK PATIENTS
We look for gaps in care and fill them in; resulting in less missed opportunities for care.
DECREASED OFFICE MANAGER WORKLOAD
Leave the referrals and coordination between other providers to us. CCM takes care of that.
IMPROVED QUALITY MEASURES
This is the goal of CCM/TCM. Its effective implementation makes this a reality.
MORE QUALITY REFERRALS
Better outcomes equals better trust in your community and amongst your peers - resulting in more referrals.
CCM is designed to save money from unnecessary readmissions, which then gets funneled to you as reimbursement for a job well done.
Using a secure and highly-rated platform, we offer and assist with the following...
**Reminder: CCM codes can only be applied towards qualified Medicare patients - patients with 2 or more Chronic conditions.
Let me help you fill in the gaps and account for the care you already give.
It's as easy as 1-2-3.
Will there be a difference in your patient outcomes and bottom-line if you implement CCM, or no? How will it impact your practice? Is it worth the trial?
Once in the affirmative, sign a simple agreement for contractual services, provide a list of qualifying clients to begin services on, and let me go to work getting consents and managing their care for you.
30 days later...
Receive your summary reports on care provided and time spent. Review and approve notes. Biller applies CCM-appropriate codes and sends off to Medicare. Save all notes for future auditing.

We are so grateful for all you have done for the town of Knightdale. I really appreciate all your hardwork and dedication to the local businesses."
Quinta is the best. Out of the hundreds of nurses I know, there are only 2 or 3 in her category. If there was a disaster taking place in my facility, she would be the one I want handling it."
I needed a nurse for my father as he was getting off home health but still required more care. Finding Quinta turned out to be an answer to prayer. She coordinated his care with his doctors and his aides, helped me decide on the appropriate next steps for care, even helping to find resources that Dad needed. She always kept me in the loop. Dad ended up passing at home just as he had wished. My last months with Dad were exactly as we both wanted all thanks to Quinta.
Thanks to Quinta, my family was able to navigate a really tough spot we hit when it came to caring for two of our elderly family members. I'm trying to keep things private here but you need to know that if you need help figuring out what to do with your loved ones who are getting older, nurse Quinta is who you need to reach out to.
When I met Quinta, even though she was struggling, I always knew there was something special about her. She has worked so hard to overcome all the obstacles and heartaches to accomplish so much. America needs more people like her!"
No, Chronic Care Management is only for those Medicare patients who have 2 or more chronic conditions.
No. You don't. I do all the work on my end using a CCM-specific system. All the notes and summaries either get uploaded to whichever EHR system you use, or you have access to view and download all the records you need.
I use a top-rated and secured system. In addition, more specific security measures are implemented to ensure patient information is always safe and protected.
As with all Medicare programs, the patient has an $8 co-pay, which I go over with them when I review and obtain the consent for the program.
No. You don't pay me anything until you bill and get paid by Medicare for any patients whose care I managed through the CCM program.
No. I am never your employee. The CCM program guidelines come from the CMS. They direct how it needs to be implemented. I sign a contract with you as an independently contracted business.
As soon as your desire for improved outcomes will permit.
There are none. Absolutely none. Caution does need to be applied to ensure patient information is safe and CCM is effectively affecting quality outcomes in the positive.
Overall, the effective application of CCM improves your patient outcomes, reduces workload on your staff, and increases your revenue so you can continue offering quality care in your community.
